How Bottle Nipple Flow Rates Are Labeled
Slow, medium and fast are manufacturer terms rather than measured standards, which is why a stage one nipple from two brands can deliver very different flow.

Bottle nipples are sold in stages labeled slow, medium and fast, and parents reasonably assume those words mean something consistent. They do not carry a shared measurement behind them.
There is no common scale
Flow labels are set by each manufacturer for its own product range. A stage one nipple describes the slowest option that brand sells, not a defined rate.
Because ranges differ, the slowest nipple from one line can move milk faster than a middle option from another. The numbers compare within a brand only.
Researchers who have measured flow across products have found wide variation, including between nipples carrying the same descriptive label.
What actually determines flow
Hole size is the obvious factor, but nipple shape, wall thickness and firmness all change how much milk moves for a given amount of suction.
The vent system matters too. A bottle that admits air poorly builds vacuum inside, which slows flow regardless of what the nipple is rated at.
Milk itself varies. Thicker liquids move more slowly through the same opening than thin ones, so the same nipple performs differently with different contents.
Why the stage numbers suggest a schedule
Packaging often pairs stages with age ranges, which implies that moving up is a developmental step in the way that solid food introduction is.
Feeding is not organized that way. Many babies feed comfortably on a slow nipple throughout the bottle-feeding period without ever changing stage.
The age printed on a package is a marketing convention layered onto a product line, not a guidance document from a pediatric body.
Why flow rate gets clinical attention
Swallowing is coordinated with breathing, and the pace at which milk arrives affects how easily an infant manages that coordination.
Feeding specialists therefore consider flow when they assess an infant who struggles with bottles, alongside positioning, pacing and the baby's own history.
That assessment is clinical work. An infant who coughs, pulls away, or tires during feeds is a reason to contact the pediatrician rather than to experiment with nipples.
How to read a package sensibly
The useful information on the package is the brand's own ordering: which nipple in this line is slower than which other one in the same line.
Comparisons across brands require watching an actual feed rather than reading labels, since the words are not defined against each other.
Manufacturer instructions also cover replacement and inspection, since nipples change as they age. Those instructions belong to the product and vary between them.
